Client Care Coordinator

Home Instead of Harford/Cecil is actively seeking a Client Care Coordinator to join our team in providing in-home care services for aging adults, helping to keep them at home safely and independently as long as possible.

Responsibilities:

• Responding to service inquiries and performing in-home care consultations with potential clients and family members
• Introducing clients to their new CAREGivers
• Ensuring high quality care with regular home visits
• Collaborating with families to create a tailored service plan that best serves the client, both initially and ongoing, to ensure satisfaction and retention
• Building and nurturing relationships with community referral providers to coordinate care and drive referrals

Qualifications:

• Experience in the health care (preferably senior care) industry
• A valid driver’s license, reliable transportation and valid auto insurance
• Passion for working with seniors and helping others
• Excellent listening, verbal and written communication skills
• Ability to work independently and proactively
• Proficiency in computer technology
