Delegating Nurse - Home Care

Home Instead is seeking a dedicated and compassionate part-time Registered Nurse (RN) to join our team serving the Mid-Eastern Shore.
The RN will provide leadership, coordination, and professional services to seniors through an interdisciplinary approach.
The RN will lead our team of caregivers to provide the highest level of care to our in-home clients.

Key Responsibilities:

• Travel to see in-home clients across the 5 Mid-Shore Counties (Talbot, Dorchester, Caroline Queen Anne’s and Kent).
• Work in collaboration with clients, their families, and caregiving team to conduct initial assessments of clients' medical, social, and functional needs.
• Monitor and evaluate clients' health conditions, ensuring proper documentation and communication to resolve any open issues.
• Conduct regular quality assurance home visits to assess client well-being and address any concerns or changes in condition.
• Coordinate with healthcare professionals, including physicians, therapists, and other providers, to ensure continuity of care.
• Provide support and guidance to care professionals, offering training and resources as needed.

Qualifications:

• Valid RN license in the state of Maryland
• Minimum of 5 years of nursing experience, preferably in home health or a related setting.
• Experience in case management and care coordination.
• Strong assessment and critical thinking skills.
• Excellent written, verbal and interpersonal abilities.
• Compassionate and patient-centered approach to care.
• Ability to work independently and as part of a team.
• Proficiency in computer and related technology
• All employees must participate in criminal and motor vehicle background checks, as well as urine drug screens.
